Her Life

Her Life

December 19, 2012

Merlinda was born in the Pasay City in the Philippines on June 25, 1955 as the eldest of three living children to Ereneo Esto Esq. and Dr. Julita Abordo. She developed a passion for music at a very early age, in fact so early, teachers were apprehensive to give an insistent 3-year old her lessons. It wasn’t until she was 6 before a teacher agreed to give her piano lessons. Little did they know that her love for music was such that throughout her lifetime, she would also learned to play the guitar, harp, clarinet, melodica, accordion, banduria, ukulele, mandolin, organ, xylophone, marimba, and angklung. However, her first love will always be the piano.
Diligent and studious, Merlinda remained at the top of her class from elementary all the way through high school, and remained in the top 10 of her class through medical school. On April 20, 1977, She married Filemon (Pat) Patacxil. Inspired by her love for children, she set up a practice specializing in pediatrics in her hometown of Daet in the province of Camarines Norte in the Philippines. A year later, Philemon (Vann) was born in Manila.
After Pat and Merlinda emigrated to the U.S. in 1984, she traded her stethoscope to take on the full time job of being a mother raising her children. Wyzscx Merfil was born on September 6, 1985 in Loma Linda, California. Since then, the family made several homes throughout the U.S. including Wagner, South Dakota for two years, and eventually to Virginia Beach, Virginia. It was there that fate allowed her to stumble upon an old set of instruments from Indonesia called the Angklung. After arranging a few melodies in a sequence that allowed even non-musicians to be able to read music, it did not take long before her passion for music was rekindled with new fire, and her first Angklung group was created.
Finally, the family moved and made Tallahassee Florida their current home since 1991 and there, Czyril Johann (CJ) was born on December 28. While continuing to raise all three of her children, Merlinda stayed active with her community. She consistently gave of herself from simply accompanying people at church to arranging and conducting the Angklung Group. Later on, she decided to continue her education into computer science, eventually leading her as the office manager for the family business.
Aside from amassing a large shoe collection, playing Sudoku, she enjoyed spending time with her husband whether it was working together or traveling abroad. Merlinda was a great mother, a dedicated wife, and one of the most giving and caring friends anyone could ever have.  And though she is no longer with us, she leaves behind precious memories, a tune in our hearts, and a legacy of love that we all will cherish forever more.

Obituary

December 19, 2012

Merlinda Esto Patacxil, 56, of Tallahassee, Florida passed away on April 27, 2012 in Gainesville, Florida from complications of a stroke. Born June 25, 1955 in Pasay City, Philippines, she graduated valedictorian of her high school class, attended Philippine Union College, and continued on to pursue a medical degree at Manila Central University.   She then served as a pediatrician before moving to the U.S. in 1985. A resident of Tallahassee since 1991, she was an active member of the Tallahassee First SDA Church and shared her musical talent with the church as well as the local Filipino community. She most recently served the community as office manager for her husband's medical offices in Tallahassee and Madison.

Merlinda is an independent and vibrant lady.  As a woman of many talents, she and her husband served their community as a doctor and businesswoman; avidly contributes her musicianship to her church and family; and is a dedicated wife and a proud mother of 3 boys.

On the morning of Friday, April 13th, "Merlynne" suffered a stroke that resulted in her being admitted to NeuroIntensive Care at Shands hospital in Gainesville Florida.

Her many talents, dedication to her family and friends, and love of music will be missed by all who know and love her.

She is survived by her husband of 35 years, Filemon Patacxil, her 3 sons, Vann, Merfil, and CJ, and countless friends around the world. A celebration of Merlinda's was held on May 27, 2012 at 3 PM at the Tallahassee First SDA Church.

This has been such a great shock to us considering that I was in your office on Thursday night talking about your plans for Costa Rica.
You have been such a wonderful friend since you moved to Tallahassee
many years ago. You are such a dynamic, vibrant and energetic person so it is very difficult to imagine you under these circumstances.
We are praying for your quick recovery. Hurry up so you can enjoy Costa Rica and the Mediterranean cruise you planned. Meanwhile, may God be with you every step of the way. We love you.

To Merlynne,

I’m just one of the many that are praying for you. God is good and merciful and He knows what’s best. Imagine if this happened during your anniversary trip to Costa Rica.

I’m wishing for your quick recovery because I know that there are things ahead that you’re looking forward to. But once you have recovered, please take things a little bit slower and enjoy the time that God has given.

In this crazy world that we live in, somehow it seems that the earth’s rotation has gone a little faster or maybe we just got older. I’m also praying for Pat and your children because I know what they’re going through because I’ve been there. God listens to one earnest prayer but when there’s a multitude of earnest prayers, He notices how important the person is that’s being prayed for. You are a very important person that’s why we are all praying for you.

My Mom, Amparo Galang was Merlyn's first grade teacher at Sampaloc SDA Church Elementary School.I was then a first grader too at PUC Elem school. Many nights at the dinner table Mom would talk bout this very "bright" student that she has. She described her as very cute, the littlest girl in a small class. She said that she almost didn't have to teach her anything new as she knew everything that even a 3rd grader needed to know. When Mom was teaching other students their ABC's and 123's she was already reading the Reader's Digest and doing multiplications.I met Merlyn early on in one of their several school programs. Mom assigned her to memorize a loooong poem , "Between 2 Angels" and she memorized it to her the very the next day, then delivered it with actions and heart rending emotions in one of those school events.

Sampaloc SDA School had a very good Rondalla Group headed by Mr Ricafrente. She was the youngest member of the group. In one of their performances in one political rally, Merlyn sat at the very front looking only at the conductor as her nimble little fingers of her left hand flew across the frets of her banduria, a 14 stringed instrument. A revered political figure was in the audience, the Vice President of the Philippines, Vice President Pelaez who was only looking at the tiny girl in the very front. "Surely", he thought, "she is just pretending to play music, this piece is too fast for her to hit the notes right." So, he got up from his chair, approached the group playing and put his ears right next to Merlyns instument and discovered with delight that she was playing the lead and didn't stop until the rendition was over. What followed next was a thunderous applause and a photo op with the Vice President.Oh I would love to see that picture again. She knew how to play all the instuments in the rondalla. You name it, she knows how to play it, banduria, octavina, guitar, piccolo, even that gigantic base. But no, the Prof would not let her do it because she had to stand on a chair to play it. She later played the accordion at school events and camping trips. She would sit at the edge of the chair and sway her body to maneuver the heavy instument. She used exotic chords and she played in triads and octaves making the instrument sound like a symphony. She has perfect pitch, and truly a gifted gift from God.

Merlyn: always excelled in MUSIC, loved MEDICINE, and the greatest MOTHER to 3 lovely sons.

We love you dearly. God Bless you and all your loved ones.

Tony and I just read the latest posting with anxious anticipation.

Encouraging report! It has been what we neuro ICU nurses term a roller coaster ride but Merlinda is staying - right on target - for that much anticipated wake up call.

Please be sure you guys eat and get a bit of exercise in the cool fresh air, to keep your strength up and help with stress tolerance.

We continue to pray for Merlinda and each one of you.

Know Merlinda likes scarves so I am knitting her a really pretty one. What we knitters term a prayer scarf where the knitter prays for the recipient while knitting. Read between the lines - burning the midnight oil on this project.

Hugs!

Birthday

August 27, 2012

Written Jun 25, 2012 from CaringBridge.org

Today marks Merlyn's birthday, born 57 years ago to the day. She should still be editing and posting pictures up on facebook of her smiling with dad, enjoying warm beaches and cool volcanic mountains of Costa Rica. She was so excited planning her wedding anniversary get away only two months ago, but sadly, it's not to be.

She should be celebrating with us, probably at The Olive Garden, kicking back and sipping on a glass of sweet Roscato, her one and only favorite red wine. Or she could have scheduled a long date, for just the her and dad, taking today off work so they could go out of town, perhaps a quiet weekend in the beautiful and fascinating mountains of Tennessee. It could have been another marker of their blissful 35 years of life together, a retreat away from this troubled world. Unfortunately, it can't and would not happen, ever again.

And after this week, I could imagine she would be so busy preparing for the Mediterranean cruise, making sure to bring all the "just in case" items. She had bought dad and herself a new camera. It would always be strapped around her neck anticipating any moment worth capturing. Travel guide books to Barcelona, Paris and Rome she had previously ordered are now arriving, even basic French and German tapes for dad! She was expecting him to learn and "minimize" miscommunication problems that might occur in places we would be visiting. Spanish and Italian are similar so she was confident that as long as dad was with her, it should not be a problem.

As dad said,"her memories continue to grip my soul with mixed joy and sorrow. Yes, we had a lot of spats, but each time the storm was over, our propinquity was even sweeter and closer as the years went by. Her music will always be a rhythm that vibrates within me, for she taught me to appreciate and learn more than what music I already had."......Reminiscing her short life full of music and laughter is a soothing balm for a grieving soul like mine."

Her memories will remain with us. God granted us almost 57 years of her life to be a blessing to those she had lived and associated with, and 35 years of which, she dedicated herself to be the best wife and a mother to three boys and sister of two siblings. And together she had carefully interwoven music with love in our whole being, of which we will cherish the rest of our lives

Celebrating Merlinda's Life

August 27, 2012

Written Jun 9, 2012 from CaringBridge.org

As I stepped out of the car, I'm greeted by beams of sunlight gently piercing through clouds as each ray brilliantly shimmers off the summer moisture in the air. Meanwhile, a somewhat tropical breeze smoothly caresses the trees next to the lofty church.  Today is the day. Today is HER day.

We were at Tallahassee First SDA, the home church where Merlinda  was a pianist for over 20 years. The redbrick structure was adorned with luxuriant plants and beautifully arranged flowers. As the guests entered the lobby, they were welcomed by two cheerful ladies handing out the program for today's musical tribute.  Behind them were five enlarged pictures of Merlinda on which everyone (even guests from as far as Oregon, California, Nevada, and Virgina) autographed with endearing messages.  Next to them was a small display reverently showcasing Merlinda's mandolin, clarinet, and accordion of over 40 years.

The beautiful and elegant urn rested on a table just below the stage.  To the left was an 18x24 picture of Merlyn smiling at the base of a beautiful Alaskan mountain range. Next to it, a delicately arranged vase of flowers.

At 3:15 p.m., the program started with a prayer, and proceeded with clips from the family video collection with musical renditions by Merlinda projected on the big screen.  Further tributes featured piano solos, a trumpet solo, and the Angklung choir by the Bamboo Chimes of Tallahassee. The eulogy and memoirs were eloquently and adoringly delivered to the audience's intense attention and delight. There were also slides showing Merlinda's photos with family and friends through the years. With beautiful mixtures of tears, joy, grief, sorrow, awe, and many laughs, the program had just about every dynamic of human emotion present.

As one visitor commented, the day " was a touching, poignant family moment, not only for the immediate family, but for the extended family and friends.  If Merlyn could only hear and see, she would have been popping her buttons.  She would have been so proud at how her boys have turned out to be talented, smart, young men.  Pat, you are fortunate to have such help as you go into the later years of your life". (FAR)

Following the program, outside the church was the butterfly release...sponsored by Phillippine Union College Academy class of 1972, in which Merlyn was a member. Then a small reception was held at the Church Hall in which Filipino-American dishes were served and within the crowds further pleasantries were exchanged regarding Merlinda's life, as well as new acquaintances and friendship formed.

As the crowd started dispersing a few more friends and visitors hang around the church yard and continued to talk about the service and snapshots of old and new friends were taken: A few of them commented "It was the most beautiful and memorable memorial service we've ever attended!"

We'll never forget that day... April 27, It has been a month since mom's ascent. Now, the 27th of May, Memorial Day weekend, It was both somber and joyous, full of tears and laughter.  The last shimmering beams of sunlight slowly cascade down the distant horizon. We can all take a little breather now, but the hardest part, I'm afraid is still yet to come.  It's ok though, mom's urn is buckled into the seat, and the family's at the house. "Let's go back home mom, they're waiting on us :)"

We would like to extend our gratitude to all of you, our family, and our friends.  The burden of losing our beloved Merlinda is a very difficult weight to carry.  For the many shoulders we've cried on, the letters of sympathy, and for the thousands of ways you've shown us how much you cared, thank you.

The Big Question

August 27, 2012

Written May 21, 2012 from CaringBridge.org by Filemon Patacxil

It's been three weeks since my beloved Merlyn is gone. I've been silent as she is.

My son Vann (Philemon) who has been the main writer and editor of the CaringBridge diary urged me to come out of my shell. In reality, I've not been in hiding because I found that in solitude there is not only peace but solace as well. I've been mulling over and looking for reason for my existence without my partner.

Adam was sad when he was alone in the garden of Eden. But God saw his sadness, and He gave him Eve to live with him for a long time. The difference is, He gave me Merlyn for a short time. Why?

But thanks, to Him, he has given me three musical boys (like their mom) who are my comfort and fortress especially at this time in my life. They have even become closer to me. Our Tallahassee home thus far is still their home.

I realize He has blessed us also with friends who understand our family's loss. Flood of cards, phone calls, emails, text messages, personal visits, still continue to come in and this alleviate the grief that we suffer. We feel His omnipresence and I feel His spirit move in us trying to tell me something why this had to happen.

Adam was lonesome
And God gave him Eve..
He suddenly took my Merlyn,
Now I'm alone, I can't believe.

There has to be a reason
There has to be a rhyme,
I think I'll just write some verses,
I think He'll answer in due time..

Break Bread Together

August 27, 2012

Written May 14, 2012 from CaringBridge.org

Like Merlinda, some of us grew up in a small community. If you're one of them, well, then you'll know that when somebody is successful, we all take part, but you'd also know that in tragedy, we all pull closer together as a family...

This past Saturday, the Filipino-American Big Bend Association of Tallahassee, held a special mass at the Good Shepherd Catholic Church. Reverend Foley led the mass in ritualistic prayer in honor of her spirit transcending whilst offering words of comfort. Sitting at the the front pew was "Merlinda" accompanied by her family.

The gathering was overflowing with expressions of love, sympathy, and encouragment for one another. Everyone also had also the chance to touch Merlinda's urn.

Of course, no Filipino get-together is complete without food so the crowd went for a delicious brunch at the Village Inn. There the association reflected about Merlyn and her contribution to the Big Bend Asso. as an officer and musical exponent.

Afterwards, we took mom to Orlando for a mini-reunion with some of her high-school friends and classmates. Some even coming from as far away as California. Here, we come across another Filipino gathering, the host prepared, you'll never guess, a LOT of food aside from the several OTHER Filipino dishes brought in by the group, including some of Merlyn's favorites and it was delicious!

And as tradition goes, it was time for pictures! We had several poses with Merlyn, even one with a friend playing a grand piano with mom sitting on the bench next to her. As the sun set and the night deepened, the atmosphere turned nostalgic and lightened up as we watched some home videos.

It's been only two weeks since Merlyn departed from among us friends and loved ones. Though Mother's Day weekend began a bit somber, it was through fellowship and reverent prayer that helped lift some grief of loss, and it ended as another day to remember her and break bread together.

A Warm Welcome

It's almost two weeks now since mom's departure, our family pet Morris the cat decided to join her today as well. At least she's got some fuzzy company for her spiritual journey into the cosmos.

Today, we were blessed with a token of her presence. CJ and I took a trip to Gainesville to pick up mom's urn and bring her back. We lovingly buckled the ornamental jar in the back seat for the ride... I guess like she did when we were kids. It took a while to get use to mom in her easily portable form, but we had a good two hour car ride to get reacquainted.

Dad greeted her with open arms as we welcomed her back into the home we all grew up in.

Announcing the Celebration of Merlinda's Life

August 27, 2012

Written Apr 29, 2012 from CaringBridge.org

Merlinda's family would like to express their gratitude in the outpouring of support we've received since her hospitalization. We have been asked by many regarding services for Merlinda. Her wishes were to be cremated, so instead of a traditional funeral, we will be holding a service in celebration of her life. There are no specific details yet, but it will be held in the next 3-4 weeks in Tallahassee, Florida. Please check back on this website or on any of the Patacxil family member's facebook page for further details.

Once again, we graciously thank everyone for their generous support.

Hello everyone, we have scheduled a service to celebrate Merlinda's life, to be held on Sunday, May 27, tentatively at 3 PM, at the Tallahassee First SDA Church in Tallahassee, Florida. Please stay in touch for further details, and again we thank you for your continued support!

Merlinda's Ascent

August 27, 2012

Written Apr 28, 2012 from CaringBridge.org

Throughout the evening while Merlyn continued further into her deep slumber, many candles burned, prayers never ceased, and emails incessantly arrived in hopes of some kind of miracle.

Her ICP (intracranial pressure) stayed over 100 (20 or less is considered normal), which is incompatible with life. After receiving another two units of packed red cells, her cheeks became rosier, and her lazy eyelids drooped even more beautifully. She has been a sleeping beauty for a couple of weeks since the beginning of her hospitalization.

She did not pass her neurological exam yesterday, and the final test today would either prove or disprove that result. The blink, corneal and caloric tests were negative. Both of her pupils were dilated and non-reactive. Finally, the apnea test was done. She registered a blood carbon dioxide level of over 70 (45 is considered normal). The doctor, who is the head of the Critical Care Team unit, had the difficult task of delivering the bad news. However, he instantly became our compassionate, consoler, adviser, and empathizer.

One of Merlyn's friends brought a pastor who talked about the mystery of life that we lose in this world, and the hope of redemption. After the short service, we gathered around Merlyn and at the request of her family, her life support was unplugged. Prayerfully, we sang Amazing Grace and witnessed as her mighty heartbeat began to slow down on the cardiac monitor. From then, every remaining beat echoed of her love as a wife, her devotion as a mother, and the music she carried in that heart that touched the lives of many in this world. At 6:26 pm. the last heart beat was seen, followed by a straight line. The sun was about to set and Merlyn has gone to rest.

The Final Stretch

August 27, 2012

Written Apr 26, 2012 from CaringBridge.org

Today, the world feels upside-down and the uneasy feeling of emptiness begins to set in; Knowing that Merlinda, devoted wife and mother of 3 boys, peacefully slumbers and may never wake up, at least in this lifetime. Her face reflects of a fruitful and content life with the spirit to take on challenges of the next greatest adventure.

The journey up to this point though was a tough and difficult one for everyone. But, if you know Merlynne, you’d know that she has a knack of making the difficult look easy, and the easy look beautiful; And that, she did, even though her ICP reached critical levels multiple times throughout the night. This afternoon, Merlynne was taken away to perform a cerebral blood flow study (an initial confirmatory test to determine the brain’s viability and the chances of a functional recovery). Final testing will follow when all trace of sedatives pass and all the electrolytes are in normal range so as to be sure the final results are conclusive without bias. Only a miracle can prove that human knowledge and technology can be wrong.

Despite the doctors bringing back the devastating results, the whole family showers her with nothing but love and adoration at her bedside. The I-pod plays a symphonic medley of her favorite music next to her ear as the caretaker removes her sedating medications one by one. She introduced all of her children to music by placing headphones on her belly before we were born in this world, it seems only fitting that we give her music as she quietly leaves and takes the cue for her curtain call.

The Last Thread of Hope

August 27, 2012

Written Apr 25, 2012 from CaringBridge.org

This entry is by far one of the hardest ones to write. Medical science has only come so far. The doctors, working hard with their wisdom and diligence have reached the peak of medical and surgical intervention... After the craniectomy, she showed some signs of improvement. However, it was short lived. When the ICP started climbing, it somehow became intractable or resistant... even to maximum medication. Despairingly, her doctors gave us a very unsettling and grim prognosis, but we still cling to some hope like a miracle from above.

Back to Surgery (Addendum)

August 27, 2012

Written Apr 25, 2012 from CaringBridge.org

It's been over 12 hours since the decompressive craniectomy. Other than receiving 2 units of blood, most of the day has been rather quiet. She did have only a single bout with elevated ICP levels; a drastic improvement from the previous days. We hope she gets well rested this evening for tomorrow is another day. Hopefully then, enough of the sedatives will have passed through her system so we can work on her reflexes and sensitivities.

Back to Surgery

August 27, 2012

Written Apr 24, 2012 from CaringBridge.org

If it ever seems like you take a step forward and three back, last night would be the perfect example, and it was rough for Merlyn. She received more treatment than usual, because of elevating brain pressure (her ICP was more than twice the expected high normal). In comparing today’s CT scan from yesterdays, it has not very impressively gotten worse. She is obviously showing more adverse effects of her medications; Which could potentially create additional problems, including slowing down her natural healing process.  Once again, it was decided that surgical intervention was needed.

A little before 9 am today, she was taken to the OR for an emergency decompressive craniectomy, (a neurosurgical procedure which a portion of the skull is removed to allow a swelling brain to expand without being squeezed). Its immediate advantages include: 1) quicker relief of intracranial pressure (ICP) without too much medications, hence fewer side effects 2) easier maintenance of normal ICP would enhance quicker brain healing from the strokes that she has had. This is a much simpler procedure than the previous one.

Hopefully, if we don’t see any further complications, weaning her off some of the medications could start as early as tomorrow, as previously planned. Cases of strokes like Merlyn's show signs of improvement between 10-20 days. No one can predict so we watch over her continually day to day.

We are fortunate that the trained Neuro-ICU nurses are very diligent and efficient in taking care of Merlyn and other seriously ill patients 24/7. They are very knowledgeable and able to answer most questions when the attending doctors are not readily available.

Progress

August 27, 2012

Written Apr 23, 2012 from CaringBridge.org

It's been 11 days since Merlinda was admitted (10 days now since her surgery). Her neurosurgeon reviewed Friday's CT scan of the brain and her progress over the weekend. He said most of the brain swelling was around the initial site of stroke and appears to be dissipating slowly. The residual clots that were not accessible for removal during surgery contribute to the slow progress as well as causing the vasospasm/ constriction of the arteries (blood being an irritant when found outside the blood vessels). He ordered another scan today with contrast and another interventional angiography if needed. The result showed definite improvement so the angiography was NOT necessary. This is definitely a good progress AMEN to that!   He then recommended that she be continued with the sedatives and get checked again in two days.  Although there are long term adverse effects of these drugs (e.g.memory), discontinuing them prematurely would put her at risk for another potential stroke and greater brain damage.  We don't want either of them to happen but we need to do what is best in her interest. Natural healing is a indeed a slow process. She had quite a few bouts of elevated intracranial pressure  ( 5-8 times) daily for the past 4-5 days. She should maintain a normal intracranial pressure without bouncing up to where she could have another stroke. She gets treated by i.v. medication each time the brain pressure go up and she had been responding beautifully. It is expected that she will have less and less surge of intracranial pressure in the next few days.....perhaps Wednesday...we can't wait!  She still has some swelling on her right eye, and fluid retention on her 4 extremities which is expected due to her i.v. fluids and other medications.  We are hopeful, and glad to see progress towards her recovery.

Tranquil Weekend

August 27, 2012

Written Apr 21, 2012 from CaringBridge.org

Saturday and Sunday (Day 9 &10 into the stroke)
After the morning rounds on Saturday , we were told that most likely there wouldn't be any intervtentional angiography as everything was stable.. Merlynne stayed stable in her drug induced coma state.
There is, however, a possibility that they will start weaning her off the sedatives beginning early in the week. Looking forward to Monday morning. We miss talking to you mom.

A Bittersweet Moment

August 27, 2012

Written Apr 20, 2012 from CaringBridge.org

It has been relatively quiet for the past 24 hrs except for the few bumps. An increased intracranial pressure readily came down in minutes with medical intervention at bedside. A CT scan of the brain was stable with no further evidence of a new stroke or exacerbation of brain swelling. For this reason, no interventional radioangiography (IR) was planned for the day.

It was a relief to hear this kind of progress knowing how dangerous that procedure can be. She continues to be on the critical list for close watch. However, close monitoring came with a small price.  The amount of blood work required for her care has made her become quite pale and anemic. So she received two units of packed red cells and almost instantaneously, she turned pink.

Today also marks Pat and Merlinda’s 35th anniversary.  Today was suppose to be full of warm sunshine, cool white sand between the toes, and the invigorating smell of the Costa Rican sea breeze.  I guess we can’t always get what we want, or go where we planned.  But at the root of it all, mom and dad planned on being together; the trip can wait, but at least they’re together.

Another Day to Rest

August 27, 2012

Written Apr 19, 2012 from CaringBridge.org

Merlynne rested well throughout the night, although unresponsive due to heavy sedation. A recurring vasospasm of the anterior communicating artery emerged, but readily responded to radioangraphic intervention using intravenous Verapamil. Once again, she was hooked up to an EEG monitor and fortunately, it showed no evidence of seizures.  The Continous Intracranial Pressure monitor showed intermittent elevation controlled with medical therapy. She had stable vital signs and we've had a wonderfully uneventful day.

Post Surgery Day 4

August 27, 2012

Written Apr 18, 2012 from CaringBridge.org

Today was another full day of rest for Merlynne.  A follow-up angiogram showed a persistent but stable brain edema. A new vasospastic vessel (the anterior communicating artery) was found but readily responded to medical angioplasty.
As much as we would love for her to wake up right now, she continues to be under full sedation to reduce the risk of another vasospasm or exacerbation/extension of present brain edema.
Tomorrow is another Day.

Post Surgery Day 3

August 27, 2012

Written Apr 17, 2012 from CaringBridge.org

During her routine neurological response check in the early morning, she was hardly responsive to commands; She was not moving her left hand/arm and leg but had muscle twitching. The attending neurosurgeon resident feverishly worked her up thru most of the morning and early afternoon for over 6 hours. A CT scan showed a big non-hemorrhagic stroke close to the prior stroke and also brain swelling causing a midline shifting of the brain.
Although given prophylactic medications, the dreaded vasospasm was suspected and an interventional radio angiogram confirmed the suspicion for which a successful medical angioplasty followed. The vasospastic vessel was a big portion of the middle cerebral artery, causing a big stroke and brain edema (swelling); full sedation and ventilation were done, and constant neurologic and ICP (intracranial pressure) monitoring with electrodes were implemented. The attending neurosurgeon said the vasospasm came on early but not unusual; neurologic deficit could not be determined at this point.
Though her progress had an expected setback, she slowly but surely continues on her path to recovery.

Post Surgery Day 2

August 27, 2012

Written Apr 16, 2012 from CaringBridge.org

Merlinda began her morning somewhat restless. An increased dosage of Propofol among several medications helped to keep her calm and rest. However, by late evening, she began losing strength on left upper and lower extremities.

Post Surgery Day 1

August 27, 2012

Written Apr 15, 2012 from CaringBridge.org

Obviously very swollen on the right side of her face, especially around eyelids. She did however respond well to command although lethargic each time she was checked. Otherwise, she slept quietly most of the day.

Surgical Intervention

August 27, 2012

Written Apr 14, 2012 from CaringBridge.org

After her stabilization last night, Merlyn slept in the next 15 hrs occasionally awakened for neurological check.  However, around 3 am this morning, she became restless and coughed up her leaking endo-tracheal tube.  About 6 or 7 Intensive care unit personnel rushed in to attend to her.

Since she was breathing on her own and maintaining other normal vital signs, she was left alone without replacing her ET tube, albeit, her oxygen was continued. At this point, her senses were returning, but still a bit confused. Though she did not know where she was, she knew who she was.

By mid morning, her memory was steadily returning as recalled the date, and where she was, but was unsure how she got to Gainesville questioning how and why. She also remembered that she had a planned vacation to Costa Rica that following Tuesday for her wedding anniversary trip, and that she and her classmates will go for a reunion cruise in the Mediterranean in July.Anxiously, she also asked why she was going to have surgery and  repeatedly asked when it was going to be done so she could go home.

At noon, she was wheeled to the OR. Surgery started at around 1:15 PM, and after over 4 hours of nervous waiting, she was wheeled back to her room, intubated with a bandage around her head . She responded groggily to simple commands, moving her fingers and toes.

Her bleeding aneurysm was located at the posterior inferior cerebellar artery on the right hemisphere of the brain. Her world renowned surgeon and his team of resident neurosurgeons clipped the artery to permanently stop the bleeding and prevent recurrence. Most of the clots were evacuated. However, they had warned of a potential complication, vasospasms, which could lead to (non-hemorrhagic) strokes.

Life Flight

August 27, 2012

Written Apr 13, 2012 from CaringBridge.org Friday started off less than normal as Merlynne, while still in bed, began her day noticing an unusual and progressive tingling on the right half of her face and body. Even at this state she asked 911 to be called, and was rushed to and received immediate medical attention at the Capital Regional Medical Center in Tallahassee. A CT scan of the brain, demonstrated a hemorrhagic stroke, and her ER doctor made an expeditious arrangement  for her immediate transfer to Shands Hospital in Gainesville.   At this point, she was in a comatose state and seriously needing intervention.  On arrival to William Shands hospital, after less than 50 minutes of flight, a shunt from the brain was immediately inserted to decompress intra-cranial pressure . The anticipated effect of the shunt placement would stabilize her in preparation for surgery the next day for brain aneurysm.
On an neurological scale called a Glasgow, she scored a 3 (indicating deep unconsciousness). She was also intubated to assist in her brain oxygenation.
Waiting for signs of improvement was agonizing for those who immediately came to see her at the NeuroIntensive Care Unit.
