Keith was born in Enid Oklahoma on May 23, 1974 to Neil and Carol Vaverka Chermack and died Wednesday July 9, 2014 at St. Mary's Regional Medical Center in Enid.

Keith graduated from Waukomis High School and Enid Beauty College.

He is survived by his parents Neil and Carol Chermack of Waukomis; three brothers Neil Chermack, Jr. of Lexington, Joe Chermack and wife Vickie, Craig Chermack and wife Patricia all of Enid; four nieces Elizabeth, Seara, Kaitlyn, Haileigh, four nephews Camron, Jakub, Evan, Joshua; his maternal grandmother Helen Vaverka and many loving aunts and uncles.

Keith was preceded in death by his paternal grandparents Joe and Lillian Chermack and maternal grandfather Rudolf Vaverka.

In lieu of flowers memorial donations may be made in his name to the St. Joseph's Building Fund in Hennessey or charity of choice with the funeral home serving as custodian of the fund.(Cordry-Gritz Funeral Home, Hennessey.)
