I am very sad to hear about Grahams death, my thoughts are with his family and friends. Graham will be missed by the department and his contribution to education and healthcare on both personal and professional levels are a tribute to him.

Graham had a very big impact upon how I practise medicine and his advice proved invaluable in finding a specialty and Consultant post which have proved exceptionally rewarding and fulfilling for almost thirty years. You are in our thoughts at this sad time.

I am very sorry to hear of Graham's death and my condolences go to all the family. Graham was a great support to me on the PRISM study which ran from 2009 to 2011. I will always remember him as a wonderfully wise man who remained very approachable and down to earth and was always willing to help.

Dear Graham, You were an extremely knowledgeable and incisive medico-legal expert. Your astute clinical judgment was matched by real kindness towards claimants (who frequently need this) - a rare combination. Dr Neale was my tutor in 2009-2010. He always had fantastic stories to tell our group and helped us to really understand how to help patients by paying attention to details. He wrote an excellent reference for my application for my BSc course. I am so grateful for all his help. My thoughts are with his family at this time. He was a wonderful man.

I got to know Graham quite well over the last 10 years, meeting him first when I worked at the NPSA. He was a fine man with a real commitment to his patients, his students and the NHS. I've no doubt he has inspired many young professionals over the years who will remember his wise counsel well.

Graham was a great friend since 1963; a superb & dedicated clinician; an excellent teacher, researcher & writer; a devoted husband, father & grandfather; & a courageous, inspiring patient with a drawn-out, progressive disease. He will be sorely missed and will always be an inspiration as to how a life should be lived & how to face the end with courage and equanimity. Farewell, dear friend!

I didn't know Graham very well but was always struck by his incredible intelligence, sharpness but also his humility when talking to others. He was a great man and who made a huge contribution in many ways.

I only knew Graham through email starting 1 1/2 ago. But, he seriously rocked! The work he did around the response of the law to clinical error, his endeavours to encourage a different approach (and his input into developing the NHS Redress Act), and his indefatigable energy - proved and continue to prove, inspirational to me. He was a great intellect and kind and generous with his time.

Graham was an inspirational figure for those of us who were just beginning to face the problems with patient safety. He was a leading member of our patient safety research unit and a pioneering researcher in this field. He will be fondly remembered by us all for his dedication in improving the safety of patients. He was ahead of his time in openly admitting to the errors in patient care.

I only met Graham when he came to the SITraN Open Day. His joie de vivre, and enthusiasm for life and his courage and stoicism when facing the devastating effects of MND were readily apparent. Fiona gave him tremendous love and support throughout his illness and must I am sure have been a great comfort to him. Please accept my sincere condolences following your sad loss.

Graham was a truly wonderful colleague and I will miss him greatly. Even once he stopped working, he would come and visit. I used to look forward to his visits and listen intently to his wisdom. He stood out in his compassion for students and colleagues and always had more to give. I will always remember him with respect and admiration.

Graham was an inspiration. He was a great story teller and would use these stories as a reminder of the importance of proper care for patients. To the very end he was making helpful suggestions via email to help me with my research project. He will be hugely missed at Imperial. Grahams dedication to improving patient safety was inspiring and I feel very lucky that I had the opportunity to work with him. He was a kind, caring and generous man and always went out of his way to help me with both work and non-work related matters. Graham touched the lives of so many people and his presence at the CPSSQ will be sorely missed and always remembered.

Graham broke the mould (& my tooth when teaching me to swim under water!). He was broad minded, very understanding, slightly left-field, and, all round, a genuine caring person. No pretence. 'Caring','compassionate', 'supportive' Graham was all of these things. His determination to succeed was evident from an early age and throughout his life he strove to do his very best in all he undertook. A keen Arsenal supporter and much loved and respected family member he will be fondly remembered by all those whose lives he touched - friends, family and patients aike.

From a patient perspective, Graham was in a class of his own. He did not tick boxes. Instead, he was a medical super sleuth, giving individual attention to each patient, regardless of his time and personal effort, often into the night, until the correct diagnosis was achieved. His passing is a sad and a devastating loss to medicine.

I remember Graham so well over many years. At Hammersmith Hospital in 1963, I succeded him as HP to Chris Booth and I was so impressed by his wonderful reputation with his patients. I have kept in contact over the years and recently I was so impressed to observe his courage inthe face of such a terrible illness. He was a wonderful man

May I express my sadness in hearing about Graham's passing. I was a speech therapist that worked with him in Watford and fondly remember our discussions about technology and mobile "apps" and increasing his knowledge of everything techny. When I wrote theBBC play that began AVMA Graham was the very first doctor I met who stood up against negligence. I was pretty naive. I could not believe so many doctors would hide behind a wall of silence. Bless you Graham - there was no lip service - you really did put the patient first. Graham was both a superb physician, and the most inspiring teacher I ever had the pleasure of meeting and knowing. His enthusiasm for everything he undertook was exemplary. It was simply a priviledge to have known him both as a friend and head of Medicine. While he was with us in Dublin he was an enormous help in the evolution of our Medical School.

I had the pleasure of working with Graham in 2009/10. His pleasure of teaching students was clear to all. He generously shared his experiences and work on patient safety and moved us all so much closer to recognising harm in the way we need to if we are to make the progress we all wish for. Healthcare is richer for him and I am thankful and privileged to have known him.

Graham's modesty and caring, his ability to forge ahead in the face of adversity, his enthusiasm to learn and teach, will live on in many of his ex-students and patients and in his daughter Fiona who has exchanged with him so much caring. Graham was a hugely impressive teacher and doctor and I really enjoyed working with him . He struck me as being exceptionally kind to everyone with whom he came into contact.He was particularly modest and considerate and seemed to go out of his way to find information , resources and ideas to help his colleagues, students and patients.

One of the first things that struck me about Graham was his incredible mix of charisma and caring. I have always felt that he was such a great person and I feel lucky to be one of the people who got to work with him and benefit from his knowledge and insight. Graham was a true gentleman, exceptional teacher and pioneering gastroenterologist who has inspired a generation of doctors. His pearls of clinical wisdom, always delivered with humility and a twinkle in his eye, will be recounted to the next generation by those fortunate enough to have been influenced by him. You always have those few teachers in your life, who you can never ever forget and Dr Graham is certainty somebody who I will always remember for the rest of my life. He was a fantastic teacher; his enthusiasm in teaching medicine to his students was just so awe-inspiring; he would always have so many medical stories to tell us &I would always look forward to them and feel really inspired.

Graham has always been one of my heroes, introducing me to my first patient as a medical student in Dublin (1979). His wonderful combination of achievement, humility, integrity, humour and a clear moral compass led to an all too brief encounter with the fog and ambiguity of medical practice in Dublin. His legacy is extraordinary, and our sympathies are with his family at their great loss.
